The preview seems to display properly in Rec Pro.
What does this mean exactly?

It's a good sign that it's not a VS4, camera or cabling issue; it's likely something to do with the particular installation of Wirecast or the Windows system. To know for sure, I recommend at least making sure that all windows updates are installed and also uninstalling/re-installing Wirecast.

A more thorough procedure is as follows:

1. Uninstall VS4 driver

2. Uninstall Wirecast

3. Backup important data then Run CCleaner (temp files and registry cleanup). https://www.ccleaner.com/

4. Update graphics, audio, and mainboard chipset drivers by downloading the latest drivers from the manufacturer website

5. Set Windows UAC setting to 'never notify' and Restart PC: http://support.microsoft.com/kb/975787

6. Re-download VS4 driver version 3.1.0.12622: http://www.matrox.com/video/en/support/downloads/
Verify that the full file size was downloaded. Unblock the file by right-clicking it then, under Properties --> General, click on the 'UNBLOCK' button. If there is no 'UNBLOCK' button, move on to the next step.

7. Re-download and re-install Telestream Wirecast

8. Install VS4 driver

Then launch Wirecast and test VS4 inputs.
